Spleen Size Measurement

Clinical Implications

  • In patients with suspected portal hypertension or cirrhosis, splenomegaly is a sensitive but nonspecific sign that should be routinely reported, according to the American Association for the Study of Liver Diseases 1
  • For accurate assessment of spleen size in cirrhotic patients, imaging is necessary as part of calculating the LSPS score (liver stiffness × spleen size/platelet count) which helps predict clinically significant portal hypertension, as recommended by the American Association for the Study of Liver Diseases 1, 2

Hematologic Disorders

  • In hematologic disorders, particularly myeloproliferative neoplasms, accurate spleen measurement is critical for disease staging, treatment response assessment, and monitoring disease progression, according to the European Society for Medical Oncology 3
  • A standardized scoring system can be used when palpating the spleen to improve consistency, with scores ranging from 0.5 (normal spleen) to 4 (spleen extends to the right rib cage), as described in Nature Protocols 4, 5
