Mechanism of Action and Clinical Applications of Tranexamic Acid

Pharmacodynamic Effects and Clinical Applications

  • In vitro studies have suggested that a concentration of 10 μg/mL is required to inhibit fibrinolysis, which is relevant for patients with abnormal bleeding due to local or systemic hyperfibrinolysis 1
  • The antifibrinolytic effect of TXA makes it useful in trauma-related bleeding, preventing accelerated fibrinolysis, as recommended by clinical guidelines for trauma care 1, 2

Important Cautions and Contraindications

  • The American College of Clinical Pharmacology contraindicates TXA in disseminated intravascular coagulation (DIC) as it may worsen the condition by inhibiting the breakdown of widespread clots, with a strength of evidence level of high 3
