Bacterial Vaginosis and Trichomoniasis Treatment Guidelines

Introduction to Treatment

Bacterial Vaginosis Treatment Regimens

  • For nonpregnant women, the first-line oral therapy is metronidazole 500 mg orally twice daily for 7 days, with cure rates of 78-84% 1, 2
  • Alternative regimens for nonpregnant women include clindamycin cream 2% intravaginally at bedtime for 7 days, with a cure rate of 82% 1, and metronidazole gel 0.75% intravaginally twice daily for 5 days, with a cure rate of 75% 1
  • The American College of Obstetricians and Gynecologists recommends that pregnant women use metronidazole 250 mg orally three times daily for 7 days to minimize fetal exposure while providing systemic coverage for possible subclinical upper tract infection 3, 2

Trichomoniasis Treatment Regimens

  • The Centers for Disease Control and Prevention recommends treating trichomoniasis with metronidazole 2 g orally as a single dose, achieving cure rates of 90-95% 4, 5
  • Alternative regimens for trichomoniasis include metronidazole 500 mg orally twice daily for 7 days, with a cure rate of 89% 4, 5

Special Considerations

  • The American College of Obstetricians and Gynecologists recommends that metronidazole is contraindicated in the first trimester of pregnancy, but after the first trimester, pregnant women can be treated with metronidazole 2 g orally as a single dose 4
  • For HIV-infected patients, the same treatment regimens as HIV-negative patients can be used 2, 4, 5

Follow-Up and Partner Management

  • The Centers for Disease Control and Prevention recommends that routine follow-up is unnecessary for patients who become asymptomatic after treatment, but if symptoms persist, re-evaluation for treatment failure or reinfection is necessary 4
  • For trichomoniasis, partner treatment is mandatory, and all sexual partners must be treated simultaneously with the same dose to prevent reinfection 4, 5
