Next Line Treatment for NSCLC After Progression on Paclitaxel/Carboplatin

Treatment Recommendations

  • The National Comprehensive Cancer Network recommends adding immunotherapy to the chemotherapy regimen for patients who have progressed on paclitaxel/carboplatin without immunotherapy, even without knowing the PD-L1 status 1, 2
  • Adding an immune checkpoint inhibitor can significantly improve outcomes for patients who have progressed on chemotherapy alone, regardless of PD-L1 expression level 3, 1

Specific Treatment Options

  • The American Society of Clinical Oncology suggests adding pembrolizumab to the current platinum-based regimen (paclitaxel/carboplatin) for significant survival benefits compared to chemotherapy alone, with response rates of 58.4% versus 35.0% in squamous NSCLC 1, 2, 3
  • The combination of platinum-based chemotherapy and immunotherapy can be effective regardless of PD-L1 expression levels 1, 4

Considerations for Specific Histology Types

  • For non-squamous NSCLC, pemetrexed-based combinations are preferred over gemcitabine or docetaxel-based regimens, according to the European Society for Medical Oncology [6, 7 is ignored in favor of 6]
  • For squamous NSCLC, pembrolizumab plus carboplatin and paclitaxel (or nab-paclitaxel) is the preferred regimen, as per the National Comprehensive Cancer Network 3, 1

Treatment Duration and Monitoring

  • The European Society for Medical Oncology recommends continuing immunotherapy until disease progression or unacceptable toxicity 6
  • Regular imaging assessments (typically every 6-9 weeks) are suggested to evaluate treatment response, according to the American Society of Clinical Oncology 1, 2
