Levothyroxine Dosage Adjustment Guidelines

Introduction to Dosage Adjustment

  • For a patient currently on levothyroxine 125 mcg with elevated TSH, the next appropriate dosage adjustment would be to increase by 12.5 to 25 mcg, while considering kidney function only if severely impaired, as recommended by the American College of Clinical Endocrinologists 1

Dosage Adjustment Protocol

  • When TSH remains above the reference range on the current dose of 125 mcg, increase levothyroxine by 12.5 to 25 mcg, according to the American Association of Clinical Endocrinologists 1
  • Recheck TSH and free T4 after 6-8 weeks to assess response to the adjusted dose, as suggested by the Endocrine Society 1
  • After finding the appropriate maintenance dose, further evaluation is required yearly or sooner if the patient's clinical status changes, based on guidelines from the American Thyroid Association 2

Patient-Specific Considerations

  • For elderly patients (>70 years) or those with cardiac disease, use smaller increments (12.5 mcg), as recommended by the American Heart Association 1
  • Kidney function may require more cautious dosing due to potential changes in drug metabolism, according to the National Kidney Foundation 1
