Ceftriaxone Dosage for Endocarditis

Dosage Recommendations by Pathogen and Situation

  • The European Society of Cardiology recommends a dosage of 2 g/24 h intravenously as a single dose for the treatment of endocarditis 1, 2, 3
  • For penicillin-susceptible streptococcal endocarditis, the recommended dosage is 2 g/day intravenously or intramuscularly as a single dose for 4 weeks for native valve endocarditis and 6 weeks for prosthetic valve endocarditis 4, 5
  • For streptococcal endocarditis with relative penicillin resistance, the recommended dosage is 2 g/day intravenously or intramuscularly as a single dose for 4 weeks, combined with gentamicin 3 mg/kg/day intravenously or intramuscularly as a single dose for 2 weeks 5

Empiric Therapy before Pathogen Identification

  • The European Society of Cardiology does not recommend ceftriaxone as the primary option for empiric initial therapy of endocarditis, instead recommending ampicillin 12 g/day intravenously in 4-6 doses plus gentamicin 3 mg/kg/day intravenously or intramuscularly as a single dose 1, 2, 4, 8

Special Considerations

  • Ceftriaxone is suitable for outpatient parenteral antibiotic therapy due to its once-daily dosing 7, 8
  • The dosage should be adjusted in patients with impaired renal function 5
  • In patients with penicillin allergy, ceftriaxone should be used with caution due to possible cross-reactions, and vancomycin may be an alternative 5

Monitoring

  • Regular clinical monitoring and blood cultures should be performed to assess treatment response 8
  • When using combination therapy with aminoglycosides, renal function and aminoglycoside serum levels should be monitored 5, 8
