Treatment of Sudden Onset Akathisia

Immediate Management Algorithm

  • For sudden onset akathisia, immediately reduce the antipsychotic dose or switch to a lower-risk agent, and simultaneously initiate propranolol 10-30 mg two to three times daily as first-line pharmacological treatment, according to Praxis Medical Insights 1
  • Reduce the current antipsychotic dosage while maintaining therapeutic range, or switch to an antipsychotic with lower akathisia risk such as quetiapine or olanzapine, as recommended by Praxis Medical Insights 1
  • Avoid antipsychotic polypharmacy, which increases side effect burden, as noted by Praxis Medical Insights 1
  • Akathisia is frequently misinterpreted as psychotic agitation or anxiety, leading clinicians to inappropriately increase antipsychotic doses, which worsens the condition, according to Praxis Medical Insights 1
  • Propranolol is the most consistently effective treatment for acute akathisia, as stated by Praxis Medical Insights 1
  • In patients with high cardiovascular risk, carefully consider QT-prolonging effects when switching antipsychotics, as advised by Praxis Medical Insights 1

Special Considerations for Acute Presentations

  • Children and adolescents are at higher risk for extrapyramidal side effects including akathisia compared to adults, according to Praxis Medical Insights 1
  • SSRI-induced akathisia is associated with increased suicidality, and clinicians should be alert to this relationship, as reported by the American Academy of Child and Adolescent Psychiatry 2
  • Systematically inquire about suicidal ideation before and after treatment initiation when akathisia occurs with antidepressant therapy, as recommended by the American Academy of Child and Adolescent Psychiatry 2
