Treatment of Herpes Esophagitis

Introduction to Treatment

  • The Centers for Disease Control and Prevention recommends a dosage of 400 mg of aciclovir orally five times a day for 10 days or until clinical resolution is achieved for patients with herpes esophagitis 1

Considerations for Treatment

  • For immunocompetent patients with herpes esophagitis, the standard treatment is 400 mg of aciclovir orally five times a day for 10 days 1
  • In cases of severe symptoms or in patients who cannot tolerate oral medication, intravenous treatment with aciclovir at a dose of 5-10 mg/kg every 8 hours for 5-7 days or until clinical resolution is achieved should be considered 2
  • Immunocompromised patients, especially those with HIV, may require longer treatment courses as healing can be slower 3

Alternative Regimens Based on Disease Severity

  • For mild to moderate disease, 400 mg of aciclovir orally five times a day for 10 days can be used 1
  • Alternatively, 200 mg of aciclovir orally five times a day can be considered, although bioavailability is lower 3

Special Considerations

  • Patients with HIV or who are immunocompromised should be closely monitored as they may require longer treatment courses 3
  • The aciclovir is primarily excreted through the kidneys, so dose adjustment is required in patients with renal insufficiency 1

Warnings and Precautions

  • The American College of Physicians notes that aciclovir does not eradicate latent virus nor affect the risk, frequency, or severity of recurrences after the medication is stopped 3
  • High doses of aciclovir require monitoring of renal function and neutrophil count 1

Patient Guidance

  • Patients should avoid close contact while they have active lesions to prevent transmission, as advised by the Centers for Disease Control and Prevention 4
