Management of Constipation in Patients Taking Anticholinergics

Prevention Strategy

  • The American College of Physicians recommends implementing prophylactic laxative treatment for all patients starting anticholinergic medications, with stimulant laxatives such as senna or bisacodyl as first-line treatment 1
  • The National Comprehensive Cancer Network recommends increasing fluid intake and encouraging physical activity when appropriate, as well as increasing dietary fiber intake if the patient has adequate fluid intake and physical activity level 2
  • Consider polyethylene glycol (17g with 8oz water twice daily) as a preventive measure, as recommended by the Praxis Medical Insights 1

Treatment Algorithm for Established Constipation

  • The National Comprehensive Cancer Network recommends discontinuing any non-essential constipating medications when possible, ruling out impaction, and ruling out obstruction through physical exam and abdominal imaging when indicated 2
  • Treat other causes of constipation, such as hypercalcemia, hypokalemia, hypothyroidism, and diabetes mellitus, as recommended by the National Comprehensive Cancer Network 2
  • Titrate stimulant laxatives, such as bisacodyl 10-15 mg daily to TID, with a goal of one non-forced bowel movement every 1-2 days, as recommended by the National Comprehensive Cancer Network and Praxis Medical Insights 1, 2

Special Considerations

  • Patients with Parkinson's disease on anticholinergics may benefit from fermented milk containing probiotics and prebiotic fiber in addition to standard constipation management, as recommended by Clinical Nutrition 3

Monitoring and Follow-up

  • Aim for one non-forced bowel movement every 1-2 days, as recommended by the National Comprehensive Cancer Network 2

Common Pitfalls to Avoid

  • Failing to provide prophylactic treatment from the start of anticholinergic therapy, as noted by Praxis Medical Insights 1
  • Using stool softeners alone without stimulant laxatives is insufficient for anticholinergic-induced constipation, as recommended by Praxis Medical Insights 1
