Management of Neuropathic Pain with Inadequate Response to Current Therapy

Optimization of Current Medications

  • The Mayo Clinic guidelines recommend ensuring the patient has been on therapeutic doses for at least 6-8 weeks before declaring treatment failure, with an adequate trial for duloxetine requiring 2-4 weeks at the target dose of 60-120 mg/day 1
  • If pain remains ≥4/10 after optimizing duloxetine to 120 mg daily for 4 weeks, the Mayo Clinic guidelines recommend adding a tricyclic antidepressant (TCA) as combination therapy 1

Addition of New Medications

  • Nortriptyline or desipramine are preferred TCAs due to better tolerability compared to amitriptyline, with a starting dose of nortriptyline 10-25 mg nightly, increased every 3-5 days to 50-150 mg nightly 2
  • The Journal of the National Comprehensive Cancer Network recommends adding lidocaine 5% patches as adjunctive therapy for localized pain, which can be used in combination with systemic agents (duloxetine and pregabalin) 2

Considerations for Medication Use

  • The Mayo Clinic guidelines recommend obtaining screening ECG if patient is >40 years old before initiating TCA, and limiting TCA dose to <100 mg/day in patients with cardiac disease 1
  • Gabapentin requires doses of 1800-3600 mg/day for neuropathic pain efficacy, divided three times daily, with a starting dose of 300 mg once daily, increased by 300 mg every 3-7 days until reaching 1800 mg/day minimum 3, 5

Referral and Special Considerations

  • The Mayo Clinic guidelines recommend referring to a pain specialist or multidisciplinary pain center if trials of optimized first-line medications in combination fail, including failure after adequate trials of duloxetine 120 mg + pregabalin 600 mg + TCA at therapeutic doses 1
  • The Journal of the National Comprehensive Cancer Network recommends considering referral earlier if there are complex comorbidities, substance use concerns, or severe functional impairment 2
