Tapering Duloxetine
Standard Tapering Protocol
- The American College of Physicians and the VA/DoD Clinical Practice Guideline recommend tapering duloxetine 40 mg gradually over at least 2 to 4 weeks before complete discontinuation, recognizing that concurrent fluoxetine therapy may actually provide some protection against discontinuation symptoms due to its long half-life and serotonergic activity, in patients who have been on therapy longer than 3 weeks 1, 2
- The standard approach to minimize discontinuation-emergent adverse events (DEAEs) is a gradual reduction over at least 2 to 4 weeks, as recommended by the 2020 VA/DoD Clinical Practice Guideline 1, 2
Tapering Approach and Monitoring
- A specific tapering approach for a 40 mg daily dose is to reduce to 30 mg daily for week 1-2, and then to 20 mg daily for week 3-4, before discontinuing, as recommended by the Mayo Clinic Proceedings 1
- If significant DEAEs emerge during the taper, it is recommended to slow the taper or temporarily increase the dose before resuming a more gradual reduction, according to the Mayo Clinic Proceedings 4
Pharmacological Considerations
- Fluoxetine at 20 mg daily converts approximately 43% of extensive metabolizers to poor metabolizers through CYP2D6 inhibition, maintaining serotonergic activity, as reported in the Mayo Clinic Proceedings 3
- The concurrent fluoxetine therapy may mitigate some duloxetine discontinuation symptoms because both medications enhance serotonergic neurotransmission, and fluoxetine's long half-life provides sustained serotonergic activity during the duloxetine taper, as noted in the Mayo Clinic Proceedings 3
