Tenofovir Administration Guidelines
Pre-Exposure Prophylaxis (PrEP) Regimens
- The American Medical Association recommends TDF 300 mg/emtricitabine 200 mg (Truvada) once daily as the first-line oral PrEP for all at-risk populations, with a double dose on day 1 for men who have sex with men (MSM) to achieve maximal protection within 24 hours 5, 6, 7
- For MSM, the event-driven "2-1-1" PrEP regimen is recommended, with 2 tablets of TDF/emtricitabine 2-24 hours before sexual activity, then 1 tablet 24 hours later, and 1 tablet 24 hours after that 6, 7
- TAF 25 mg/emtricitabine 200 mg (Descovy) once daily is recommended for MSM with creatinine clearance 30-60 mL/min or those with osteopenia/osteoporosis, with no loading dose required 5, 8, 9
Critical Pre-Administration Requirements
- Mandatory testing before starting PrEP includes a combined HIV antibody/antigen test, serum creatinine and estimated creatinine clearance, hepatitis B surface antigen, hepatitis C antibody, and STI screening 5, 8, 10
- Ongoing monitoring during PrEP includes every 3-month HIV testing with combined antibody/antigen assay, three-site STI screening, and creatinine clearance checks 5, 8, 10
Important Clinical Caveats
- Patients with chronic HBV switching HIV regimens must continue tenofovir (TAF or TDF) to maintain HBV suppression, with a risk of severe hepatitis flare or hepatic decompensation if tenofovir is discontinued 11, 10
- TAF dosing may need adjustment when co-administered with certain medications, and didanosine and atazanavir require dosage modification when co-administered with tenofovir 11
- Contraindications for TDF-based PrEP include creatinine clearance <60 mL/min, osteopenia/osteoporosis, and undiagnosed HIV infection 10, 9, 7
