Migraine Prevention with Topiramate

Indications and Efficacy

  • The American Academy of Family Physicians recommends considering topiramate for migraine prevention when patients experience ≥2 migraine attacks per month producing disability for ≥3 days, use of acute/rescue medication >2 times per week, failure of or contraindications to acute treatments, or uncommon migraine conditions 1, 2, 3
  • The American Academy of Family Physicians suggests that topiramate is effective in reducing migraine frequency by approximately 2 attacks per month at 100 mg/day, and 44.6% of patients achieve ≥50% reduction in headache frequency overall 1, 2
  • The American Academy of Family Physicians notes that topiramate allows for full clinical benefit to become apparent after 2-3 months, consistent with other migraine preventive agents 1

Safety and Counseling

  • The Journal of Neurology, Neurosurgery and Psychiatry advises that topiramate reduces the efficacy of oral contraceptives and other hormonal contraceptives, requiring alternative or additional contraception 4
  • The Journal of Neurology, Neurosurgery and Psychiatry warns of teratogenic risk and the need to counsel all women of childbearing potential about pregnancy risks 4
  • The Journal of Neurology, Neurosurgery and Psychiatry highlights potential adverse effects, including depression and cognitive slowing, requiring monitoring 4

Treatment Algorithm and Alternatives

  • The American Academy of Family Physicians states that topiramate is not first-line for migraine prevention, and first-line preventive agents include propranolol, timolol, amitriptyline, divalproex sodium, and sodium valproate 1, 2, 3
  • The American Academy of Family Physicians recommends NSAIDs as first-line for acute attacks, and triptans for attacks not responding to NSAIDs 1, 2, 3, 5

Monitoring and Adjustment

  • The American Academy of Family Physicians suggests reassessing patients at 6-8 weeks after reaching target dose and continuing monitoring at least every 3 months during maintenance therapy 1, 2, 3
  • The American Academy of Family Physicians advises considering tapering or discontinuing topiramate after a period of stability to assess ongoing need 1, 2, 3
  • The American Academy of Family Physicians recommends switching to a different preventive agent rather than increasing topiramate dose if no response at 100 mg/day after adequate trial (2-3 months) 1

Migraine Prevention with Topiramate

Clinical Guidelines and Recommendations

  • The VA/DoD guidelines suggest topiramate for migraine prevention with a weak recommendation 6, 7, 8

Patient Selection and Considerations

  • Topiramate should be especially considered for patients concerned about weight gain, currently overweight, or with coexisting epilepsy 6, 7, 8
