Blood Pressure Monitoring for Cyclosporine in Psoriasis Patients

Pre-Treatment Blood Pressure Assessment

  • The American Academy of Dermatology recommends establishing a baseline blood pressure measurement on at least two separate occasions before initiating cyclosporine therapy, as uncontrolled hypertension is a contraindication to starting cyclosporine therapy 1, 2, 3

Monitoring Schedule During Treatment

  • The British Association of Dermatologists and FDA labeling strongly recommend measuring blood pressure every 2 weeks during the first 3 months of cyclosporine therapy, when the risk of hypertension development is highest 1, 2
  • The American Academy of Dermatology recommends measuring blood pressure monthly if stable after the first 3 months of cyclosporine therapy 1, 2

Critical Monitoring Technique

  • Early morning resting blood pressure is a more sensitive indicator of early nephrotoxicity than elevated creatinine, due to cyclosporine-induced renal arteriole vasoconstriction 1, 4

Management Thresholds and Actions

  • The American Academy of Dermatology recommends reducing the cyclosporine dose by 25-50% if sustained hypertension develops (≥140/90 mm Hg on 2 separate occasions) 1
  • If blood pressure does not normalize after multiple dose reductions, the American Academy of Dermatology recommends discontinuing cyclosporine 1

Antihypertensive Selection

  • The American Academy of Dermatology recommends calcium channel blockers as the antihypertensive agents of choice, as they counteract cyclosporine's vasoconstrictive mechanism by relaxing vascular smooth muscle 1, 4, 5
  • Specifically recommended agents include isradipine, nifedipine, felodipine, or amlodipine 1, 4, 5
  • The American Academy of Dermatology also recommends β-blockers for blood pressure control 1

Special Considerations for High-Risk Patients

  • Impaired renal function is generally a contraindication to cyclosporine initiation, but if treatment is considered essential, extremely close monitoring is required 2

Incidence and Timing of Hypertension

  • Cyclosporine causes hypertension in approximately 20% of patients, with the incidence being dose-dependent 4
  • Hypertension can develop at any time during treatment, but first elevated blood pressures are typically recorded early after starting therapy (median: 1 month) 4

Integration with Other Monitoring

  • Blood pressure monitoring should be coordinated with renal function assessment (serum creatinine and BUN every 2 weeks for first 3 months, then monthly) 1, 2
