Initiating Semaglutide 2.4mg Therapy for Weight Management

Eligibility and Contraindications

  • The American College of Cardiology recommends starting Semaglutide 2.4mg if the patient has a BMI ≥30 kg/m² or BMI ≥27 kg/m² with at least one weight-related comorbidity, provided there are no contraindications 1, 2
  • Patients with a personal or family history of medullary thyroid cancer, multiple endocrine neoplasia syndrome type 2 (MEN2), pregnancy, or breastfeeding should not start Semaglutide 2.4mg 1, 3

Dosing and Administration

  • The dosing schedule for Semaglutide 2.4mg should be titrated slowly over 16 weeks, starting with 0.25 mg subcutaneously once weekly and increasing to 2.4 mg once weekly by week 17 1, 3, 5

Lifestyle Interventions

  • The National Institute for Health and Care Excellence recommends combining Semaglutide 2.4mg with a caloric deficit of 500 kcal/day, minimum 150 minutes/week of physical activity, resistance training, and behavioral counseling for sustained lifestyle changes 2, 3, 5

Expected Outcomes

  • Patients can expect a mean weight loss of 14.9-16.0% of initial body weight at 68 weeks, with 86.4% achieving ≥5% weight loss 3

Common Side Effects

  • Gastrointestinal effects such as nausea (44%), diarrhea (32%), and vomiting (25%) are common, but typically mild-to-moderate and transient 1, 4, 3

Monitoring and Discontinuation

  • Patients should be monitored every 4 weeks during titration and every 3 months after reaching maintenance dose, with discontinuation considered if weight loss is <5% after 12 weeks or significant safety issues emerge 1, 2, 3

Long-Term Considerations

  • Semaglutide 2.4mg is a lifelong therapy, with weight regain occurring after stopping, and patients should plan for indefinite treatment to maintain weight loss and health benefits 3

Cardiovascular Benefits

  • Semaglutide 2.4mg provides a 20% reduction in cardiovascular death, nonfatal MI, or nonfatal stroke in patients with obesity and established cardiovascular disease 3
