Venlafaxine Dosing and Administration

Standard Dosing Algorithm

  • The therapeutic efficacy range of 150-225 mg/day typically requires 2-4 weeks to achieve, as recommended by the Mayo Clinic 1, 2, 3, 4

Special Populations

  • No specific dose adjustment is required for elderly patients based on age alone, but exercise caution and use slower titration when increasing doses, as suggested by clinical guidelines 1, 2, 3, 4

Context-Specific Dosing

  • When used as a coanalgesic for neuropathic pain, the starting dose is 37.5 mg once or twice daily, with titration to increase by 75 mg each week, and a target therapeutic dose of 150-225 mg/day, as recommended by the Mayo Clinic and the Journal of the National Comprehensive Cancer Network 1, 2, 3, 4, 5
  • The maximum dose for neuropathic pain is 225 mg/day, and the duration of an adequate trial is 4-6 weeks at therapeutic dose, as suggested by clinical guidelines 1, 2, 3, 4

Important Clinical Considerations

  • Blood pressure monitoring is essential, particularly at doses >200 mg/day, as dose-dependent blood pressure increases can occur, as recommended by the Mayo Clinic 1, 2, 3, 4
  • Cardiac conduction abnormalities have been reported in a small number of patients, and venlafaxine should be prescribed with caution in those with cardiac disease, as suggested by clinical guidelines 1, 2, 3, 4
  • Always taper when discontinuing venlafaxine, as withdrawal syndrome has been described, and abrupt discontinuation should be avoided, as recommended by the Mayo Clinic 1, 2, 3, 4
  • Venlafaxine is available in both immediate-release and extended-release preparations, allowing for once-daily dosing and potentially improving tolerability, as suggested by clinical guidelines 1, 2, 3, 4
