Initial Treatment and Management of Parkinson's Disease
Pharmacological Therapy
- The American Academy of Neurology recommends levodopa/carbidopa as the initial treatment for patients with Parkinson's disease, as it is the most effective medication for controlling motor symptoms 1
- Levodopa/carbidopa is the most effective treatment for motor symptoms, including tremors, and should be initiated at the time of diagnosis or soon after 1, 2
- Taking levodopa at least 30 minutes before meals avoids interactions with dietary proteins that reduce absorption and efficacy 1, 2
- Implementing a protein redistribution diet (consuming most proteins at the end of the day) improves motor function in patients with motor fluctuations 1, 2
Management of Non-Motor Symptoms
- Melatonin is the recommended treatment for REM sleep behavior disorder, especially in older patients 1
- Clonazepam is commonly used but has side effects: morning sedation, gait imbalance/falls, depression, and cognitive impairment 1
Advanced Therapies for Refractory Symptoms
- Deep brain stimulation (DBS) of the subthalamic nucleus (STN) and globus pallidus internus (GPi) are equally effective for controlling motor symptoms 3, 4
- STN DBS is chosen when the primary goal is to reduce dopaminergic medications 2, 3, 4
- GPi DBS is chosen when there is significant concern about cognitive decline (especially processing speed and working memory) 1, 2, 3, 4
- GPi DBS is also chosen when there is concern about the risk of depression 3, 4
- Both targets are equivalent for improving quality of life 3, 4
Emerging Therapies
- Magnetic resonance-guided focused ultrasound (MRgFUS) is an emerging non-invasive option for medication-resistant tremor 2
Nutrition and Lifestyle
- Avoiding inappropriate dietary restrictions, such as strictly low-protein diets, which can cause nutritional deficiencies without evidence-based benefits 1, 2
- Monitoring for nutritional deficiencies, as increasing doses of levodopa are associated with a higher risk of malnutrition, requiring careful monitoring 1, 2
