Anticoagulation Management in Artificial Heart Valve Patients
Target INR Range
- The American Heart Association recommends a target INR range of 2.5-3.5 for patients with mechanical mitral valves (Class I recommendation) 1, 2, 3
- For patients with mechanical aortic valves, the target INR range is 2.0-3.0 for bileaflet or Medtronic Hall valves without risk factors 1, 2, 3
- The American College of Cardiology recommends a target INR range of 2.5-3.5 for bileaflet or Medtronic Hall valves with risk factors, as well as for Starr-Edwards or other mechanical disk valves 1, 2, 3
Risk Factors and INR Adjustment
- The presence of risk factors such as atrial fibrillation, previous thromboembolism, left ventricular dysfunction, hypercoagulable conditions, and older-generation thrombogenic valves requires a higher target INR range 3, 4
- The American Heart Association recommends considering an INR range of 2.5-3.5 for the first 3 months post-operatively, even for low-risk aortic valves (Class IIa) 1, 2, 3
Management of INR Deviations
- The American College of Cardiology recommends bridging with heparin if INR is <2.0 in high-risk patients (mechanical mitral valve) 4
- For INR values above the target range, the American Heart Association recommends withholding warfarin and monitoring with serial INRs 4
Aspirin Consideration
- The American College of Cardiology recommends adding low-dose aspirin (75-100 mg daily) to warfarin therapy in mechanical valve patients, particularly those with additional risk factors (Class I recommendation) 1, 2, 3
