Ramelteon Efficacy and Safety for Adult Insomnia
Clinical Evidence and Recommendations
- The American Academy of Sleep Medicine recommends ramelteon 8 mg for sleep onset insomnia, based on trials demonstrating efficacy, with a mean reduction in objective sleep latency of 9.57 minutes (95% CI: -6.38 to -12.75 minutes) compared to placebo 1
- Meta-analysis of ramelteon 8 mg showed that 63-66% of patients achieved ≥50% reduction in sleep latency at weeks 1, 3, and 5 with the 8 mg dose 1
- The American Academy of Sleep Medicine provides a WEAK recommendation for ramelteon due to marginal efficacy, though benefits appear to outweigh minimal harms 2
Special Considerations and Safety
- Ramelteon has minimal to no clinically significant effect on total sleep time (mean increase of only 6.58 minutes), wake after sleep onset, sleep efficiency, or sleep quality 1, 2
- No evidence of rebound insomnia, withdrawal effects, or abuse potential was observed in studies lasting up to 6 months 1, 2
- Cognitive Behavioral Therapy for Insomnia (CBT-I) should be the initial treatment before or alongside ramelteon 2, 3
Treatment Guidance
- If ramelteon 8 mg is ineffective after 1-2 weeks, consider alternative agents rather than increasing the dose, as 8 mg is the maximum approved dose 2, 5
- Ramelteon is particularly suitable for patients who prefer non-DEA-scheduled medications or those with a history of substance use disorders 3
- Do not prescribe ramelteon for sleep maintenance insomnia (frequent awakenings or early morning awakening), as it has no clinically meaningful effect on wake after sleep onset and may actually increase it by 3.5-5.2 minutes 1, 4
