Post-Influenza Lingering Cough Management

Understanding Post-Infectious Cough

  • The American Thoracic Society suggests that postinfectious cough occurs following viral respiratory infections and can persist for 3-8 weeks after the acute illness resolves, resulting from persistent airway inflammation and epithelial disruption from the viral infection, transient bronchial hyperresponsiveness without eosinophilic inflammation, and mucus hypersecretion and impaired mucociliary clearance 3, 4
  • The European Respiratory Society indicates that antibiotics have absolutely no role in postinfectious cough and should not be prescribed, as this is NOT a bacterial infection 5, 3

First-Line Treatment Approach

Home Remedies

  • The British Thoracic Society recommends honey and lemon mixtures as the initial approach for symptomatic relief, which are safe, inexpensive, and provide meaningful symptom relief without side effects 1, 2, 6
  • The American College of Chest Physicians suggests that adequate hydration and humidified air may provide symptomatic benefit, and paracetamol (acetaminophen) can be used for any residual discomfort 1, 2

Over-the-Counter Options

  • The American Academy of Family Physicians indicates that dextromethorphan is the most effective OTC cough suppressant, with standard OTC doses providing modest benefit, and higher doses (60 mg) may be more effective but require medical supervision 1, 2, 7
  • The European Respiratory Society suggests that menthol lozenges provide additional symptomatic relief by working through cold and menthol-sensitive receptors to suppress cough, and can be used alongside other treatments 2, 6

When to Escalate Treatment

  • The American College of Chest Physicians recommends considering a trial of inhaled ipratropium bromide, which may attenuate postinfectious cough, and is more effective than continuing OTC antitussives indefinitely 3, 7
  • The British Thoracic Society suggests that inhaled corticosteroids may be considered when cough persists despite ipratropium and adversely affects quality of life, and may address the underlying airway inflammation 3

Red Flags Requiring Medical Evaluation

  • The American Thoracic Society indicates that the patient should see a doctor if coughing up blood, breathlessness develops, fever returns or persists, symptoms worsen rather than gradually improve, or cough persists beyond 3 weeks 1, 2, 7

Expected Timeline

  • The European Respiratory Society suggests that post-viral cough typically resolves within 2-3 weeks without specific treatment, and if cough extends beyond 3 weeks but less than 8 weeks, it remains in the postinfectious category 7, 3, 4
  • The American College of Chest Physicians recommends that if cough persists beyond 8 weeks, consider diagnoses other than postinfectious cough and evaluate for chronic cough causes (asthma, GERD, upper airway cough syndrome) 3, 4
